At first I was gonna say to never use the burn tool to shade, but that's a render.

Your lighting is doing something mighty weird then.

To be constructive, i think the worms skin seems to pink and the hands and shading is a bit off, specially on the hands.

The lips appear as if a snake bit them or something.

i gotta admit i agree at firs it was sharp without a blur it looked like my grandmas lips
it was discusting!

shape/outline: splendid!!!
colour: looks a bit sunburnt so if you fixed that it would also be splendid!!!

also you just reversed the left but it doesn't look right, you need to should the inside palm therefore making it perspectivly correct *starts picking nose*
