How do I host or join a direct IP game?

I try using the TCP IP option to play with a friend, but the game one of us hosts never shows on the list, is there a way to enter the ip for it to connect directly?

Yes.
If you have the latest beta updates (http://wa.team17.com/main.html?page=supp&area=upda&file=15) there's a direct IP button and box on the bottom of the network game menu (the 2 computers).
The host clicks the button "Direct IP/TCP" and hosts a game as normal, the players that want to join enter the IP address of the host in the box next to it and press enter or click the button.

You can also enter DNS addresses in the box.
(I use a www.no-ip.info address because I have a dynamic IP address.)

If you can't host at all, you maybe have to configure your firewall and/or router correctly.
www.nanacide.com/wahelp has lots of info.

You have to download & install the update client too:
http://www.no-ip.com/downloads.php
It's a small tool that runs in the background (low on ressources), auto-starts with Windows and periodically checks your external IP address for changes.
If it changes, it sends the resolved IP address to no-ip.com.

If a player enters berker.no-ip.info into the box, he will always have your current external IP address.
You as host don't have to do anything.
